How are states' geographical boundaries determined on a map?

When I look at a weather map, I am fascinated and curious as to how each state looks. If one were able to lift any state off the map, would the shape of the state look the exact way as each is depicted on a map? How were the views of each continent determined before there were the tools available to study geography?
