Monroe county ny URGENT!!!!!! HELP!!!!!!!!?

i need a pdf or doc or ANY format of an employer statement from rochester ny's department of human services. i am disabled and can not work. this is not a case of just get a job. my case is going to be turned off tomorrow morning and i will not have health care, or rent help, or rg&e help etc... i need the doent tonight so i can (re) fax it to dhs. i already had it filled out and faxed, but the worker says she never got it and my case will be terminated and i will be in a HUGE dillema. PLEASE!!!!!!! if anyone knows where i can get one (on the web), i will be so gratefull. thank you
